CITY SEAL: The Seal of the City of South Bend shall be hot ( <br />1 NO <br />stamped onto both sides of the cart body. Street side of containers <br />shall be marked NO PARKING. <br />USER INSTRUCTIONS: Instructions for the safe use of the <br />YES 7, NO <br />container must be molded into each lid. Instructions shall be in both <br />" <br />English and Spanish. <br />LOAD RATING: The load rating of the container must be raised- <br />YES J%O <br />relief molded into the lid. Load rating shall be stated in both pounds <br />and kilograms and in English and Spanish. <br />EXPERIENCE: <br />YINO <br />REFERENCES: Bidder must submit with its bid a reference list of <br />municipalities currently using the bidder's products. The list must <br />include at least ten (10) municipalities who currently have at least <br />20,000 carts in service. Include the name of the municipality, year of <br />installation, contact person, phone number, and quantity for each <br />reference. Failure to include these references will result in bid <br />disqualification. <br />WARRANTY: Bidder must submit with bid a document which <br />YES //NO <br />clearly states the exact warranty of the bidder. The warranty must be <br />for no less than ten (10) full years and must specifically provide for <br />no -charge replacement of any component parts which fail in <br />materials of workmanship for a period of ten (10) years after <br />installation. The bidders warranty is understood to include, whether <br />stated in bidder's warranty or not, the following coverage: <br />Failure of the lid to prevent rain water from entering the container <br />when in the closed position. <br />Damage to the container body, lid, or any component parts through <br />'V S NO <br />opening or closing the lid. <br />Failure of the lower lift bar from damage during interface with lifters. <br />YES) NO <br />Failure of the body and lid to maintain their original shape. <br />YES NO <br />Damage or cracking of the container body through normal operating <br />Yl :1 NO <br />conditions. <br />Failure of the wheels to provide continuous, easy mobility, as <br />Y1S, NO <br />originally designed. <br />Failure of any part to conform to minimum standards as specified <br />YI"51O <br />herein. <br />If bidder is owned by another business entity, then the owning entity I <br />Y1n;S)NO <br />must also accept full financial responsibility for the warranty of the <br />bidder. The bidder must submit with their bid a letter from the <br />owning entity which clearly states its obligation and commitment to <br />
