Gibson -Lewis, LLC <br />The following forms of questionnaires and financial statement are prescribed by the State <br />Board of Accounts in confomnity with the statute set out on the proceeding page. <br />These forms, properly filled out and attested, must accompany each bid of Five Thousand <br />Dollars or more on any public work, <br />The forms am designed to cover all contracts for all lands of work and the bidder is <br />required to answer such questions as are pertinent to the work upon which he is bidding. <br />The purpose of the questionnaire and financial statement, as set forth in the law, is to <br />enable the awarding body to determine the qualifications of the bidder to carry out <br />successfully the contract if the same is awarded to him. <br />The bidder will find it to his advantage to answer fiilly all questions coming within the <br />range of the work upon which he is bidding. Particular attention should be given the <br />' Financial Statrune it " and the details relative to the assets and liabilities set out. This <br />form is made in extensive detail so that the bidder may explain his assets and liabilities in <br />Proper sequence and in a uniform manner. <br />
