EXHIBIT B: INCOME COMPUTATION AND CERTIFICATION' <br /> NOTE TO APARTMENT OWNER: This form is designed to assist you in computing <br /> Annual Income in accordance with the method set forth in the Department of Housing and Urban <br /> Development ("HUD") Regulations (24 CFR Part 5). You should make certain that this form is <br /> at all times up to date with HUD Regulations. All capitalized terms used herein shall have the <br /> meanings set forth in (or incorporated by reference in) the Regulatory Agreement and <br /> Declaration of Restrictive Covenants, dated as of April 1, 2020, between MAH CEDAR GLEN, <br /> LP ("Cedar Glen Apartments") and the City of South Bend. <br /> Re: Cedar Glen Apartments <br /> South Bend, Indiana <br /> I/We, the undersigned, being first duly sworn, state that I/we have read and answered <br /> fully and truthfully each of the following questions for all persons who are to occupy the unit in <br /> the above apartment project for which application is made.
