("Petitioner"), makes application to the Common Council <br />of the City of South Bend, Indiana to amend the zoning ordinance as herein requested. <br />1) The property sought to be rezoned is located at: <br />Lots 11, 12, 13, 14, 15, 16, 17, 18, 19 and 20 in Dinan's Second Addition to <br />the City of South Bend and the parcel of property between the South and the <br />East ends of said lots and the West end of lots 1 through 10 in said addition, <br />which entire parcel of property is more particularly described in Exhibit <br />"A" attached hereto and made a part hereof. This property is commonly <br />known as 508-536 Crescent Avenue, South Bend Indiana. <br />2) Name and address of property owner(s) of the petition site: <br />Madison Center, Inc. <br />403 East Madison St. <br />South Bend, Indiana 46601 <br />3) Name and address of contingent purchaser(s), if applicable: <br />None <br />4) It is desired and requested that this property be rezoned: <br />From: "D" Light Industrial District "f' height and area District <br />To: "A-3" Mixed Use Multifamily Residential and Commercial District <br />"f' height and area District <br />5) This rezoning is requested to allow the following use(s): <br />Medical Clinic use <br />6) Attached is a copy of (a) a legal description of the property; (b) a list of names and addresses of <br />all property owners and the tax key numbers for all properties within 300 feet of the petition <br />property; (c) six (6) preliminary site plans; and (d) addressed, stamped envelopes for all property <br />owners within 300 feet of the petition property. <br />
