CITY OF SOUTH BEND, INDIANA <br />BOARD OF PUBLIC WORKS <br />REQUEST FOR QUALIFICATIONS <br />Sealed Applications shall be received the office of the Board of Public Works, 13th Floor, County <br />City Building, Room 1316, South Bend, Indiana, for: <br />QUALIFIED CONTRACTORS FOR LEAD HAZARD REDUCTION GRANT <br />SUMMARY: <br />The Board of Public Works of the City of South Bend ("City"), on behalf of the Department of <br />Community Investment is soliciting applications from qualified vendors to serve as preferred vendors for <br />its Lead Hazard Reduction Program. <br />Project description: <br />The City of South Bend offers the Lead Hazard Reduction Program to control or eliminate lead hazards in <br />properties owned or leased by low-income households. It is the purpose of this request to establish a list <br />of qualified vendors who will perform lead hazard reduction work at the rates established herein. This is <br />for one of two grants addressing lead -based paint that the City has received that have the same <br />requirements. Vendors who are interested in participating in both grants they should indicate in their <br />application that they wish to participate in both grants. <br />The Board will open applications each Board of Public Works Meeting, Ste. 1308, County -City Building, <br />227 W. Jefferson Blvd., South Bend, Indiana 46601. Representatives of the City will review the <br />applications and notify vendors if they have been accepted into the program pursuant to the terms below. <br />Applications must meet the minimum specifications as provided herein. There is no scoring matrix <br />as all vendors whose applications meet the minimum specifications and who meet and accept the <br />following conditions will be accepted into the program. <br />Qualified Applicants must meet and accept the following conditions: <br />Requirements: <br />• A copy of the State of Indiana Abatement Contractor License must be included. <br />• Copies of all individuals valid state licenses for lead abatement workers & supervisors. <br />• One-year warranty on work <br />• Company safety manual must be followed. <br />• Must be an EPA approved Renovation, Repair and Painting certified firm. <br />• Must follow South Bend City Standards including materials <br />• Bills must be delivered to Suite 1400S, County -City Building 227 W. Jefferson Blvd. 46601 by the 5th of <br />the following month after work performed under the program <br />• Proof of Commercial General Liability Insurance per Person in the amount of $50,000; and <br />• Proof of Commercial General Liability Insurance per Occurrence in the amount of $1,000,000; and <br />• Worker's Compensation Insurance. <br />• No contract award may be made to parties listed on the government -wide exclusions in the <br />System for Award Management (SAM) or Federal Awardee Performance and Integrity <br />Information System (FAPIIS). <br />• Copies of any contractor licenses (plumbing, electrical, etc.). <br />• A list of any sub -contractors that would be used. <br />
