PRE -QUALIFICATION CHECKLIST (FOR BIDDERS THAT ARE NOT PRE -QUALIFIED) <br />(b) Attachments: <br />(ii) List identifying all former business names. <br />• Superior Construction has no former business names. <br />(iii) Superior Construction Co., Inc. received two OSHA citations in 2017, both of which were <br />quickly resolved. They were a NON -SERIOUS citation: 29 CFR 1926.20(b)(2) : The employer's safety <br />& health program did not provide for frequent and regular inspections of the jobsite and <br />equipment by a competent person; and a SERIOUS citation: 29 CFR 1926.760(a)(I): Each employee <br />engaged in a steel erection activity who is on a walking/working surface with an unprotected side <br />or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by <br />guardrail systems, safety net system, personal fall arrest systems, positioning device systems, or fall <br />restraint systems. <br />The mitigation plan for these included the following: <br />1) Competent Person training for all sites and documenting the competent person(s) on <br />each site. <br />2) Fall protection training for all sites. <br />3) Additional resources and management placed on the site. <br />4) Additional onsite safety observations required by management. <br />5) Re -structure of new employee orientation. <br />6) Also, since this incident we have had a restructure with the HSE department, new HSE <br />manual developed with new HSE operating procedures. <br />(iv) Superior Construction Co., Inc. employs a diverse group of skilled workmen and supervisors. <br />Superior focuses on preserving our workforce by supporting local apprenticeships, alignment with <br />education, relationship building, investment in training, coordination with local union trade <br />associations including Carpenters, Cement Masons, Laborers, Iron Workers, Millwrights, Operators, <br />Pile Drivers, Tech Engineers, Teamsters, Northwest Indiana Contractors and AGC of Indiana. <br />(v) Individuals who will perform work on the public work projects on Superior Construction Co., <br />Inc.'s behalf will be properly classified as an employee or as an independent contractor under all <br />applicable state and federal laws and local ordinances. <br />(vi) Superior Construction Co., Inc. is signatory with the following unions: Carpenters, Cement <br />Masons, Laborers, Iron Workers, Millwrights, Operators, Pile Drivers, Tech Engineers, Teamsters. <br />Each union Superior is signatory with is registered with the US Department of Labor. Being <br />signatory with these unions enables Superior to participate in their apprenticeship and training <br />programs. The combined on-the-job training and classroom work enables Superior to grow our own <br />talent and teach our organization's best practices. <br />
