picture theater or adult mini motion picture theater. <br /> (b) License Required. <br /> (1) No person shall operate an adult business <br /> without first obtaining a license hereunder, issued by the <br /> Controller. <br /> (2) All adult businesses existing and operating as <br /> of December 31, 1995 must submit an application for a license <br /> hereunder within ninety (90) days of said effective date. If <br /> such an application is not received within said ninety (90) <br /> day period, then such establishment shall cease operations . <br /> An adult business existing and operating as December 31, 1995 <br /> shall have until February 15, 1997 to satisfy the requirements <br /> of Sections 4-18 .5 (1) (4) , 4-18 .5 (1) (7) , 4-18 .5 (n) (2) , 4- <br /> 18 .5 (n) (3) and 4-18 .5 (o) . <br /> (3) An application for a license hereunder must be <br /> made on a form provided by the City of South Bend. The <br /> application must be accompanied by a sketch or a diagram <br /> showing the configuration of the premises, including a <br /> statement of total floor space occupied by the business . The <br /> sketch or diagram need not be professionally prepared but must <br /> be drawn to a designated scale. <br /> (4) The applicant must be qualified according to the <br /> provisions of this ordinance and the premises must be <br /> inspected by the Building Commissioner and found to be in <br /> compliance with the provisions of Section 4-18 .5 (1) , if <br /> applicable. <br /> (5) Each application for a adult business license <br /> must be signed by the applicant, if an individual, or by an <br /> authorized representative if the applicant is a corporation, <br /> partnership or other entity or association. <br /> (6) The fact that a person possesses other types of <br /> state or city permits and/or licenses does not exempt him from <br /> the requirement of obtaining an adult business license . An <br /> establishment containing at one (1) location more than one (1) <br /> operation characterized as an adult business must comply with <br /> all regulations hereunder pertaining to each such operation <br /> but must obtain only one (1) adult business license for that <br /> establishment . <br /> (7) Applications for a license, whether original or <br /> renewal, must be made to the Controller by the intended <br /> operator of the adult business . Application forms shall be <br /> supplied by the Controller. The intended operator shall be <br /> required to give the following information on the application <br /> form: <br /> a. l .
