All payments will be directed to subgrantee as follows: <br />Daniel Parker, Controller <br />227 W. Jefferson Blvd, 121h Floor North <br />South Bend, IN 46601 <br />Email: dparker@southbendin.gov <br />ARTICLE 6. PRIOR APPROVAL REQUIREMENTS <br />All requests for budget revisions requiring the Endowment's approval will be initiated by Brian <br />Donoghue and routed through Dr. Fuja to Notre Dame Research Administration, Cortney Swift, <br />Grants Program Manager. <br />ARTICLE 7. AUDIT OF RECORDS <br />Subgrantee agrees to provide Notre Dame with information as requested to meet the University <br />of Notre Dame's monitoring requirements. Each parties' records pertaining to this subgrant will <br />be open to Notre Dame and Endowment Staff as needed. Site visits by Notre Dame and the <br />Endowment staff may occur for review and documentation of project activities. No audits or <br />professional evaluations of the funds forwarded from Notre Dame may be initiated without <br />advance written approval by Notre Dame Research Administration (NDRA) and Research & <br />Sponsored Programs Accounting (RSPA). Subgrantee will provide access to financial records in <br />support of these -activities as_the_project begins -and for the duration -of-the _project. All records, <br />including financial records, supporting documents such as bills, invoices, cancelled check, <br />receipts, etc., and statistical records are to be retained for a minimum of three years after date of <br />final payment. However, if any litigation, claim or audit is started before the expiration of the <br />three-year period, then the records will be retained until all litigation, claim, or audit finding <br />involving the records is resolved and final action taken. Subgrantor, the Endowment, or any of <br />their duty authorized agents shall have full access to and the right to examine any of said records. <br />ARTICLE 8. REPORTS <br />Subgrantee will provide programmatic reports to Notre Dame describing achievement of <br />anticipated outputs and according to the schedule in Exhibit D. <br />ARTICLE 9. ASSURANCES <br />Subgrantees agree to exercise their best efforts to insure that all assurances required by the <br />Endowment are satisfied. Such assurances required of the subgrantees shall include but are not <br />limited to: <br />Debarment/Suspension <br />Subgrantee certifies by execution of this Agreement that they are not suspended, debarred or <br />ineligible from entering into contracts with any Department or other Agency of the Federal <br />Government of the United Sates or the Canadian government, or in receipt of a notice of <br />proposed debarment or suspension. Subgrantee shall provide immediate notice to Notre Dame in <br />the event of being suspended, debarred or declared ineligible by the Department or other Federal <br />
