a Atthe start of the academic year, the training company will submit to ABC a stating date, <br />enrollment forms for each student accompanied by a drivers license and proof of high school <br />graduation, a DOL form, and an NCCER Registration and Release Form <br />• At the conclusion of the academic year, the Education Contactwill submit to ABC, all sign in <br />sheets, performance profiles and written tests along with a completed Form 200. <br />a NCCER certify instructors teaching classes for Michiana Contracting in-house apprenticeship <br />program; <br />a order all craft textbooks; <br />a provide orientation materials, DOL information, NCCER credentials, and invoices, verify attendance <br />in Flashpoint, send to students and education contacts final grade reports, wage letters; <br />a ensure reports are completed and submitted as required by ABC, NCCER, and the DOL; <br />a provide support to education contacts, instructors, and students as needed; and <br />a audit the training site annually to verify that all program guidelines, regulations, and instructional <br />practices are being met. <br />Costs for the ABC Training Program to be held by Michiana Contracting are listed below. <br />Core/Level 1— Cost per Student <br />a New Student Application Fee $ 100.00 <br />a Annual Tuition $ 500.00 <br />a Textbooks $ 200.00 <br />(Instructor Textbooks - s1o%opy as needed) <br />$ 800.00 <br />Level 2 — Cost per Student <br />a Annual Tuition $ 500.00 <br />a NCCER Textbooks $ 150.00 <br />(Instructor Textbooks - slop/copy as needed) <br />$ 650.00 <br />P►ogram costs for each student are quoted as of Januarys, zos9 and may be subject to change. <br />Ing at . <br />Where program guidelines, regulations, and/ortraining conditions are not sufficiently met by <br />students, instructors, and/or the training facility, ABC of Indiana/Kentucky will drop all s u with <br />the Department of Labor and immediately terminate the training program. If the ABC <br />Apprenticeship Coordinator is required to complete any unfished responsibilities of the training <br />company in order to comply with IDOL and/or NCCER quidelines, an additional fee of s2oo per <br />student will be added to this agreement. Failure to pay the additional administrative fee will <br />result in a termination of the training agreement. <br />As additional students are added to a class, it is the responsibility of the training memberto ensure <br />ABC is notified and supplied with the appropriate information as listed above. <br />pg. 2 <br />
