AMENDED MASTER SCHEDULE <br />TO <br />SEP FINANCIAL ASSISTANCE AGREEMENT <br />SEP Recipient is: City of South Bend, Indiana <br />Effective Date shall be: , 2019; <br />Term of Agreement shall be two (2) years after the Effective Date <br />SEP Funds shall be: $10,044.00 to be authorized under this Financial Assistance Agreement. (A total of $41,400.00 <br />was available under the Settlement Agreement referenced in paragraph 1 of this Financial Assistance Agreement, but <br />only $31,775 was disbursed of the $37,500 awarded.
