Klosinski, Assistant City Engineer <br />SUBJECT: Agenda Request Form — Change Order No. 2 <br />117-026 Wastewater Treatment Plant Headworks Influent Gates Improvements <br />DATE: July 10, 2019 <br />w,�wwwiomw�!�u�twmmwwa ��rn� r ,✓re�ruarv�io�ram� urrra�arvarimuuimm �u�wvru�:r.. � araru�r�a rcrmv,�vu.nrm. nuvr�nwo�w:mmru�uur�auan��su�ov+xr�wwrnmmuurmmswunw ��grawrmm�^mA wmatr�u. uxrA n..rAr �maws.�va.w�un�n�..m �,rw m:�rAd�mm^n�u�n <br />The Division of Utilities is requesting the Board of Public Works' approval of Change Order No. 2 for <br />Project No. 117-026 Wastewater Treatment Plant Headworks Influent Gates Improvements. <br />Change Order No. 2 includes the following items: <br />COR #3: Remove Concrete Topping at Junction Chamber 1 and 2 <br />$ 14,249.00 <br />COR #4: Terminate electrical conduit and cable at Junction Chamber 2 <br />$ 756.00 <br />COR #5: Sediment Removal at Junction Chambers <br />$ 3,083.62 <br />COR #6: Addition Handrail at Stairs, Junction Chamber 1 <br />$ 2,488.00 <br />Total Increase <br />$ 20,576.62 <br />The Original Contract Sum: <br />$966,500.00 <br />Net change by previously authorized Change Orders: <br />($351,345.56) <br />The contract sum prior to this Change Order: <br />$615,154.44 <br />By this Change Order, the project amount is increased by: <br />$ 20,576.62 <br />The new Contract Sum including this Change Order: <br />$635,731.06 <br />This Change Order represents a total change of: <br />+2.13% <br />Total change for entire project: <br />-34.22% <br />Original Contracted Completion Date/Time: 10/27/2018 <br />Extension of Date/Time by Previous Change Orders:. 11/16/2019 (20 days) <br />Date/Time Extension by this Change Order: +197 Days <br />New Completion Date/Time: 05/31/2019 <br />Substantial completion was established on schedule. Seeding and restoration efforts were completed <br />in the Spring when weather was acceptable for seed application. All punch list items are complete. <br />The extension of time encompasses all necessary time for restoration efforts. This project is <br />completed with a total cost savings of $330,768.94 in comparison to the original bid price of <br />$966,500.00. Construction was completed for a cost of $635,731.06 (-34.22%). <br />The funding source for the project is Wastewater, Capital Account (642-0630-415-43-66).
