<br />PROJECT NO. 118-007 (MVH)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 2 (Final) on
<br />behalf of Rieth-Riley Construction Co., Inc., 25200 State Road 23, South Bend, IN 46614,
<br />indicating the contract amount be decreased by $14,510.87 for a new contract sum, including this
<br />Change Order, of $647,385.12. Also submitted was the Project Completion Affidavit indicating
<br />this new final cost of $647,385.12. Upon a motion made by Ms. Maradik, seconded by Ms.
<br />Dorau and carried, Change Order No. 2 (Final) and the Project Completion Affidavit were
<br />approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — LOT 12 US 31 INDUSTRIAL PARK
<br />LANDSCAPE WORK — PROJECT NO. 119-029 (RWDA TIF)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Fuerbringer Landscaping and Design, Inc., 22530 Brick Road, South
<br />Bend, IN 46628, for the above referenced project, indicating a final cost of $52,110. Upon a
<br />motion made by Ms. Maradik, seconded by Ms. Dorau and carried, the Project Completion
<br />Affidavit was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT BERLIN PLACE SITE CONCRETE
<br />LANDSCAPE PLANTINGS, AND BLDG. NO. 4 ROOF PAVERS DIVISION B — PROJECT

<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Rieth-Riley Construction Co., Inc., 25200 State Road 23, South Bend, IN
<br />46614, for the above referenced project, indicating a final cost of $325,000. Upon a motion made
<br />by Ms. Maradik, seconded by Ms. Dorau and carried, the Project Completion Affidavit was
<br />approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — LAFAYETTE BUILDING EXTERIOR
<br />MASONRY RENOVATIONS DIVISION B — PROJECT NO 117-100R (RWDA TIF)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of C&S Masonry Restoration, LLC, 3725 N. Foundation Ct., Suite AB,
<br />South Bend, IN 46628, for the above referenced project, indicating a final cost of $284,045.
<br />Upon a motion made by Ms. Maradik, seconded by Ms. Dorau and carried, the Project
<br />Completion Affidavit was approved.
<br />APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS — ONE (1) OR

<br />In a memorandum to the Board, Mr. Jeff Hudak, Central Services, requested permission to
<br />advertise for the receipt of bids for the above referenced equipment. Therefore, upon a motion
