<br />Ms. Maradik advised that Mr. Toy Villa,: Engineering, has submitted Change Order No. 3 on
<br />behalf of CBS Services, LLC, 5005 Lincoln Way E, Suite A, Mishawaka, IN 46544, indicating
<br />the project completion date be increased by thirty (30) days for a new project completion date of
<br />June 30, 2019. Upon a motion made by Ms. Dorau, seconded by Ms. O'Sullivan and carried, the
<br />Change Order was approved.
<br />APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT
<br />— 2018 DEMOLITIONS — PROJECT NO. 118-307 (COIT)
<br />Ms. Maradik advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1
<br />(Final) on behalf of Ritschard Bros., Inc., 1204 W. Sample Street, South Bend, IN 46619,
<br />indicating the contract amount be decreased by $55,341 for a new contract sum, including this
<br />Change Order, of $264,417. Additionally submitted was the Project Completion Affidavit
<br />indicating this new final cost of $264,417. Upon a motion made by Ms. Dorau, seconded by Ms.
<br />O'Sullivan and carried, Change Order No. 1 (Final) and the Project Completion Affidavit were
<br />approved.
<br />APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT
<br />— MISHAWAKA AVENUE TREE REPLACEMENT — PROJECT NO. 118-111 (COMPLETE
<br />STREETS TRANSPORTATION)
<br />Ms. Maradik advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 1
<br />(Final) on behalf of Acorn Landscaping, LLC, 3680 W. Sample Street, South Bend, IN 46619,
<br />indicating the contract amount be decreased by $2,415 for a new contract sum, including this
<br />Change Order, of $19,424.50. Additionally submitted was the Project Completion Affidavit
<br />indicating this new final cost of $19,424.50. Upon a motion made by Ms. Dorau, seconded by
<br />Ms. O'Sullivan and carried, Change Order No. 1 (Final) and the Project Completion Affidavit
<br />were approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT — FIRE STATION NOS. 10 AND 11
<br />EQUIPMENT — PROJECT NO. 118-108 (EQUIPMENT PURCHASES/MATERIALS &
<br />EQUIPMENT)
<br />Ms. Maradik advised that Mr. Toy Villa, Engineering, has submitted the Project Completion
<br />Affidavit on behalf of Herrman and Goetz, Inc., 225 S. Lafayette Blvd., South Bend, IN 46601,
<br />for the above referenced project, indicating a final cost of $47,287. Upon a motion made by Ms.
<br />Dorau, seconded by Ms. O'Sullivan and carried, the Project Completion Affidavit was approved.
<br />APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF PROPOSALS —
<br />TOWING SERVICES FOR THE DEPARTMENT OF CODE ENFORCEMENT
<br />In a memorandum to the Board, Ms. Tracy Skibbins, Code Enforcement, requested permission to
<br />advertise for the receipt of proposals for the above referenced project. Therefore, upon a motion
<br />made by Ms. Dorau, seconded by Ms. O'Sullivan and carried, the above request was approved.
<br />ADOPT RESOLUTION NO. 19-2019 — A RESOLUTION OF THE BOARD OF PUBLIC
<br />WORKS FOR THE CITY OF SOUTH BEND, INDIANA, APPROVING THE TAX-EXEMPT
<br />LEASE PURCHASE FINANCING PROPOSAL OF KEY GOVERNMENT FINANCE INC
<br />Upon a motion made by Ms. Dorau, seconded by Ms. O'Sullivan and carried, the following
<br />Resolution was adopted by the Board of Public Works:

<br />A RESOLUTION OF THE BOARD OF PUBLIC WORKS FOR THE CITY OF SOUTH
<br />BEND, INDIANA, APPROVING THE TAX-EXEMPT LEASE PURCHASE FINANCING
<br />PROPOSAL OF KEY GOVERNMENT FINANCE, INC.
<br />WHEREAS, the City of South Bend (the "City"), a political subdivision of the state of
<br />Indiana, is authorized by law to acquire and own interests in real and personal property; and
<br />WHEREAS, the City of South Bend Board of Public Works (the "Board"), on behalf of
<br />the City, has the authority under the laws of the state of Indiana to purchase, acquire, and lease
<br />property; and
<br />WHEREAS, the Board has determined that a need exists for the acquisition and financing
<br />of certain property for the benefit of the City's operations; and
