ZBeHIGHGROUPLLC <br />TE <br />An Affiliate of High Industries Inc. <br />May 1, 2019 <br />RE: Pre -Qualification Checklist — Commerce Center <br />(b) Attachments: <br />(i) Indiana Secretary of State business verification <br />(ii) List all former business names: <br />- High Concrete Structures, Inc. <br />- High Concrete Technologies <br />- High Concrete Innovations LLC <br />(iii) High Concrete Group LLC has not violated any federal, state, or local laws including but not <br />limited to, violations of contracting or antitrust laws, tax or licensing laws, environmental <br />laws, Occupational Safety and Health Act (OSHA) or federal Davis -Beacon and related Acts, <br />within the preceding five (5) years. <br />(iv) High Concrete Group LLC has 77 team members in our Springboro, Ohio facility to <br />sufficiently satisfy our production needs. <br />(v) Individuals who perform work on the Commerce Center project will be properly classified <br />as employees or contractors according to applicable state and federal laws and local <br />ordinances. <br />(vi) High Concrete Group LLC does not currently participate in any apprenticeship or training <br />programs. <br />(vii) See attachment for company drug testing policy. <br />(viii) Zurich North America Surety is the surety provider for High Concrete Group LLC. Zurich is <br />listed as an approved surety through the Bureau of Fiscal Service.
