CITY OF SOUTH BEND, INDIANA <br />BOARD OF PUBLIC WORKS <br />REQUEST FOR QUALIFICATIONS <br />Sealed Applications shall be received no later than 9:30 a.m., EST, on May 14, 2019, in the office of the <br />Board of Public Works, 13th Floor, County City Building, Room 1316, South Mend, Indiana, for: <br />QUALIFIED CONTRACTORS FOR LEAD HAZARD REDUCTION DEMONSTRATION <br />GRANT <br />SUMMARY: <br />The Board of Public Works of the City of South Bend ("City"), on behalf of the Department of <br />Community Investment and the Indiana Housing & Community Development Authority ("IHCDA") is <br />soliciting applications from qualified vendors to serve as preferred vendors for its Lead Hazard Reduction <br />Demonstration Program. <br />Project description: <br />The City is authorized under Resolution No. 4755-18 of Common Council of the City of South Bend to <br />offer this Lead Hazard Reduction Program to control or eliminate lead hazards in properties owned or <br />leased by low-income households. It is the purpose of this request to establish a list of qualified vendors <br />who will perform lead hazard reduction work at the rates established herein. <br />The Board will open the applications at n, liS lit M�:r in the Boardof Public Works <br />Meeting, Ste. 1308, County -City Building, 227 W. Jefferson Blvd., Sormth Bend, Indiana <br />46601. Representatives of the City will review the applications and notify vendors if they have been <br />accepted into the program pursuant to the terms below. <br />Applications must meet the minimum specifications as provided herein. There is no scoring matrix <br />as all vendors whose applications meet the minimum specifications and who meet and accept the <br />following conditions will be accepted into the program. <br />Qualified Applicants must meet and accept the following conditions: <br />Requirements: <br />--�- • A copy of the State of Indiana Abatement Contractor License must be included. <br />• Copies of all individuals valid state licenses for lead abatement workers & supervisors. <br />• One-year warranty on work <br />• Company safety manual must be followed. <br />��• Must be an EPA approved Renovation, Repair and Painting certified firm. <br />• Must follow South Bend City Standards including materials <br />• Bills must be delivered to Suite 1400S, County -City Building 227 W. Jefferson Blvd. 46601 by the 5th of <br />the following month after work performed under the program <br />• Proof of Commercial 1eneral Liability Insurance per- Person in the amount of $50,000; and. <br />• Proof of Commniercial (aencral Liability Insurance per Occurrence in the amount of $1,000,000; and <br />• Proof of specialized liability insurance for lead hazard reduction work. <br />• Worker's Compensation Insurance. <br />No contract award may be made to parties listed on the government -wide exclusions in the <br />System for Award Management (SAM) or Federal Awardee Performance and Integrity <br />Information System (FAPIIS) <br />• Copies of any contractor licenses (plumbing, electrical, etc.). <br />• A list of any sub -contractors that would be used. <br />
