PRE -QUALIFICATION CHECKLIST (FOR BIDDERS THAT AR NOT PRE -QUALIFIED) (b) Attachments: (i) Indiana Business Entity Report. —See attached (ii) List identifying all former business names. a. Ancon has had no other former business names (iii) Ancon has had one (1) OSHA violation in the past five years. We have had no other violations. — See attached (iv) Statement about staffing capabilities, including labor sources. This statement indicates and ensures I have sufficient employees on staff to complete the work I am bidding on OR outlines how I intend to meet the staffing needs of the work. a. Ancon has sufficient staffing capabilities on staff to complete the work I am bidding. (v) Statement that individuals who will perform work on the public work project on my behalf will be properly classified as an employee or as an independent contractor under all applicable state and federal laws and local ordinances. a. Ancon has no federal, state, or local tax liens or tax delinquencies owed to any federal, state, or local taxing body. (x) List of projects of similar size and scope of work performed in all areas, including the State of Indiana, within three (3) years prior to the date on which the bid is due: a. Since this is a unique project, Ancon has not completed any fountains in the last three years. However, Ancon has completed over 100 projects that have similar concrete work including site foundations and sidewalks. A couple of our building projects that have included concrete work includes Goodwill, South Bend Cubs, and ATC.
