Ms. Michelle Smith <br />March 18, 2019 <br />KERAMIDA Proposal No. P22579R <br />Pane 3 <br />Task 3: Technical Evaluation <br />Based on previous IDEM comments, further evaluation of MW-13 is warranted. KERAMIDA will <br />provide technical assistance to address IDEM's comments which may be received following the <br />redevelopment activities and reporting of the May 2019 sampling results. An evaluation of a <br />potential nitrate source may be required depending on the results of the 2019 sampling. Additional <br />on -Site investigation is not included in this proposal. <br />COST ESTIMATE <br />The estimated cost to complete the scope of work detailed in this proposal is $6,600 as detailed in <br />Table 1 below. All costs will be invoiced on a lump sum basis, with billing concurrent with <br />completion of each item. Other terms and conditions will be in accordance with the attached <br />KERAMIDA Standard Terms and Conditions. <br />Table 1 <br />Item _ Cost <br />Task 1: Well Redevelopment for MW-13 $1,100 <br />Task 2: Statistical Analyses & Semi -Annual Reports $4,700 <br />Task 3: Technical Evaluation for NM-13 Issue $800 <br />ESTIMATED PROJECT TOTAL 1 $6,600 <br />SCHEDULE <br />KERAMIDA will initiate work on this project immediately upon issuance of a Purchase Order <br />(P.O.) from the City of South Bend. Well redevelopment for MW-13 is anticipated to be conducted <br />one to two weeks prior to the scheduled May 2019 groundwater sampling event. Statistical analyses <br />will be initiated after receipt of analytical data for each sampling event from the City of South Bend. <br />Technical evaluation will be initiated after receiving IDEM's comments and further evaluation <br />request. <br />We look forward to the opportunity to continue to assist you with this project.
