This SUPPLEMENTAL AGREEMENT ("Agreement') is made and entered into on this date Feb 12 , by <br />and between the City of South Bend, acting by and through its proper officials, CTOCAL PUBLIC AGENCY" <br />or "LPA'� and UNITED CONSULTING, an Indiana corporation ("CONSULTANT'). <br />WI TNESSETH <br />WHEREAS, the CONSULTANT did, on April 26, 2016 enter into an Agreement with the LOCAL PUBLIC <br />AGENCY for Engineering Services and Documents in relation to the following described project: <br />Des. No, 1401810 <br />Coal Line Trail, Phase I; from Lincoinway West to <br />Riverside Drive in LaPorte District <br />This supplement is required to add the following new services to the original scope of work: <br />Phase II Environmental Site Assessment and Appraisal Problem Analysis (APA) <br />WHEREAS, in order to provide for completion of the work it is necessary to amend and supplement the <br />Engineering Agreement. <br />NOW, THEREFORE, it is agreed by and between the parties as follows: <br />L Appendix "A nis amended to read as follows: <br />E.6. PHASE II ENVIRONMENTAL SITE ASSESSMENT <br />The CONSULTANT shall complete a limited Phase II Environmental Site Assessment along <br />the Coal Line Trail. The proposed locations and spacing of the borings are based on the <br />results of the Phase I Environmental Site Assessment. <br />The CONSULTANT shall complete a total of 14 soil borings to characterize the soils at the <br />identified locations. The borings shall be advanced to depths ranging from five to eight <br />feet below the existing ground surface. Soil samples shall be collected on a continuous <br />basis at approximate 2-foot intervals throughout the depth of each boring. Each recovered <br />soil sample shall be classified and screened in the field for the presence of volatile organic <br />vapors using a portable photoionization detector (PID) and visually evaluated for obvious <br />visible indications of potential impacts. A total of 24 soil samples shall be submitted for <br />analysis. <br />Supplemental Agreement version 6-2011 <br />
