��Jl - �.wra�aw This SUPPLEMENTAL AGREEMENT (" Agreement'l is made and entered into on this date , by and between the City of South Bend, acting by and through its proper officials, ("LOCAL PUBLIC AGENCY" or "LPA'� and UNITED CONSULTING, an Indiana corporation (" CONSULTANT'). WHEREAS, the CONSULTANT did, on April 26, 2016 enter into an Agreement with the LOCAL PUBLIC AGENCY for Engineering Services and Documents in relation to the following described project: Des. No, 1401811 Coal Line Trail, Phase 11 from Riverside Drive to Angela Boulevard in LaPorte District This supplement is required to add the following new services to the original scope of work: Appraisal Problem Analysis (APA) WHEREAS, in order to provide for completion of the work it is necessary to amend and supplement the Engineering Agreement. NOW, THEREFORE, it is agreed by and between the parties as follows: L Appendix "A 9is amended to read as follows; U. APPRAISAL PROBLEM ANALYSIS The CONSULTANT shall prepare a parcel -by -parcel examination of the project site to determine the extent of each taking and the appraisal approach to be utilized by the appraiser. The CONSULTANT shall provide Indiana Department of Transportation (INDOT) approved appraisers to complete the Appraisal Problem Analyses (APA's). The APA report shall include a summary sheet setting out the following: 1. Parcel Number 2. Owner's Name 3. Size of Property 4. Amount of R/W to be acquired Supplemental Agreement version 6-2011
