Vermont <br />.3t(ems <br />Software License, Maintenance and Support Agreement <br />Amendment to Sales Agreement Dated <br />This Amendment dated �� °�`,� p is made and entered into by Vermont Systems, Inc. (VSI) <br />and (Customer), parties to the Agreement dated <br />and is amended as follows: <br />Full Hosting Services: Add or Remove Silver _ Gold _Platinum_ <br />Full VSI Hosting Services Added - the Customer is currently hosting the VSI application <br />software and Progress software on premise. Effective the Customer has requested <br />that VSI provide Full Hosting Services by installing the VSI application software and database <br />and Progress software on VSI servers at either the Eastern or Western data center, for a <br />minimum of one year. The fee for full hosting services will be included in Quote# <br />The Full Hosting Services guaranteed rate for one year includes the services and features, as <br />described in Exhibits E & F. <br />Full VSI Hosting Software Removed: VSI is currently providing the Customer with full hosting <br />services on VSI servers at either its' Eastern or Western data center. The Customer has decided <br />to discontinue using VSI hosting services. Therefore, the Customer will be responsible for <br />managing the software and database, as soon as the software installation and database transfer <br />has been completed. The scheduled date for this transfer is �. VSI wile <br />discontinue billing for full hosting services effective the last day of the transfer month. <br />Except as set forth in this Amendment, the Agreement is unaffected and shall continue in full <br />force and effect in accordance with its terms and conditions. <br />Vermont Systems, Inc.
