Michiana Contracting Statement Regarding <br />Drug and Alcohol Use in the Workplace <br />NOTE.• Currently and until further notice, the Bowen Center, whose services, for the <br />most part are covered under Michiana Contrachng's Group Insurance, will act as the <br />company Employee Assistance Program (EAP). Refer to your company handbookfor <br />further information regarding the Bowen Center. <br />Any person required by the nature of his or her duties as an employee of Michiana <br />Contracting to hold a Commercial Driver's License under Indiana Code 9-24-6, et seq., <br />shall be subject during the course of his or her employment with Michiana Contracting to <br />controlled substances testing, including as applicable reasonable cause testing, random <br />testing (as referenced above) and post -accident testing as is required by law for such <br />drivers and as a condition of his or her employment, shall cooperate in all such required <br />testing. <br />This is hereby notification that as a condition of continued employment the employee <br />will (1) abide by the terms of the above statement and (2) notify the employer of any <br />criminal drug statute conviction for a violation occurring in the workplace no later than <br />five (5) days after such conviction. <br />3 <br />
