Associated SUlyder5 <br />and contracrtor9, Inc. <br />Indiana/Kentucky <br />Chapter <br />Training Program Agreement <br />Between ABC of Indiana/Kentucky and <br />Michiana Contracting, Inc. <br />June zoi8 <br />Rationale <br />Since Michiana Contracting, Inc. is located in Plymouth, IN far from any ABC training center, <br />Michiana Contracting will implement an in-house Electrical apprenticeship training program under <br />the sponsorship of ABC Indiana/Kentucky. Atraining partnership between Associated Builders and <br />Contractors of Indiana/Kentucky (ABC) and Michiana Contracting is of great value to both <br />organizations by combining efforts to improve the construction industry through professionally <br />trained craft professionals. This written agreement clarifies the agreement between ABC and <br />Michiana Contracting to provide an in-house apprenticeship training program located at Michiana <br />Contracting, Inc., 7841 Lilac Road, P.O. Box 929, Plymouth, Indiana 46563, <br />ftr- rl101111t <br />This Training Program Agreement between ABC and Michiana Contracting is effective beginning <br />August zoj_8 and will be reevaluated in August zoig. <br />f?WrWQKkk m „} or°rsilai itie <br />Both organizations understand the necessity of submitting paperwork in a timely manner <br />and commit to meeting established submission timelines. <br />Michia aa, .onttgj t ng,will. <br />W provide classroom and lab space, equipment, and training materials that adequately support <br />implementation and continuation of the electrical apprenticeship training program; <br />■ pay salaries for in-house program instructor(s); <br />• ensure apprentices will attend class for no less than 144 hours per level of study as required by <br />the DOL. <br />• submit to the training site coordinator the following required program documents in the <br />following manner: <br />;il,m enrollment forms, performance profile sheets, employer mandated work forms, <br />i"lasl�pta-iA: log attendance weekly in Flashpoint, enter grades, OJT submission and approval <br />MaH. at the end of the semester please submit NCCER module exams and Form zoo <br />assure all rules and regulations are followed per ABC, hICCER, and the U.S. Department of Labor <br />(DOL) guidelines, <br />P9.
