Special Event and Parade Regulations Ordinance <br /> Page 17 <br /> (4) Affidavit of Applicant:The applicant must certify on a form provided by the City, <br /> by affirmation, that information provided in the application and additional <br /> documentation is true and correct; and that the applicant agrees to comply with all <br /> applicable local, state and federal regulations; and <br /> (5) Public Disclosure Information:The City recognizes that the public has a legitimate <br /> interest in being informed about special events,particularly as they directly relate <br /> to potential impacts on areas surrounding such events. Accordingly, the <br /> information provided by the applicant addressing the following shall be subject to <br /> public disclosure: <br /> (i) Event Title <br /> (ii) Event Summary <br /> (iii) Name and Contact Information for Event Organizer <br /> (iv) Event Website Address <br /> (v) Dates and Times of the Event <br /> (vi) Location of the Event <br /> (vii) Projected Attendance/Participants <br /> (viii) Participation/Attendance Fee(s) <br /> (ix) Additional Public Contacts <br /> (x) Community Outreach Efforts <br /> (xi) Fees, costs, and charges incurred <br /> (6) Non-refundable application fee of Fifty Dollars ($50.00) for Tier II and <br /> III special events,and Twenty-five Dollars($25.00)for a Tier I special event made payable <br /> to the City. <br /> (aa) Additional Requirements for Parades: If the special event is a parade, an applicant must <br /> not only include in its special event permit application the information described in Sec. <br /> 15-181(a)—(z)but also the following information:(Approximate number of persons who <br /> would witness the proposed parade along the route proposed; and <br /> (1) Date,time and estimated duration of the parade,including the time when <br /> registration is proposed to begin,the proposed time needed to organize before <br /> and disperse after the parade,and any possible alternative dates and times; and <br /> (2) Description of the route to be traveled, including the proposed starting point,the <br /> identification of and the directions to be followed on all streets,the termination <br /> point; and any areas proposed to be used to set up before and disperse after the <br /> parade; and <br /> (3) Map of the proposed route to be traveled clearly delineating all of the proposed <br /> affected streets,compass coordinates, and any anticipated street closures which <br /> may be needed; including the locations where traffic security officers will be <br /> necessary; and <br />
