<br />AWARD QUOTATION AND APPROVE CONTRACT -- FIRE STATION NO. ..3„ ROOF
<br />REPLACEMENT - PROJECT NO... 112-014 (FIRE CAPITAL)
<br />Mr, Toy Villa, Engineering, advised the Board that on March 8, 2012, quotations were received
<br />and opened for the above referenced project. After reviewing those quotations, Mr. Villa
<br />recommended that the Board award the contract to the lowest responsive and responsible bidder,
<br />Lawmasters, Inc., 1407 West Hively Avenue, Elkhart, Indiana 46517, in the amount of
<br />$62,216.00. Also presented at this time for approval was the contract, Therefore, Mr. Inks made
<br />a motion that the recommendation be accepted and the quotation be awarded as outlined above
<br />and the contract approved. Mr. Gilot seconded the motion, which carried.
<br />APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS AND TITLE
<br />SHEET — MAIN STREET /LAFAYETTE BOULEVARD CONNECTOR FROM IRELAND

<br />In a memorandum to the Board, Mr. Patrick Henthorn, Engineering, requested permission to
<br />advertise for the receipt of bids for the above referenced project. Also presented at this time for
<br />approval and execution was the Title Sheet. Upon a motion made by Mr, Inks, seconded by Mr.
<br />Gilot and carried, the above request to advertise was approved, and the Title Sheet was approved
<br />and signed.

<br />APPROVAL OF REQUEST TO ADVERTISE FOR THE NOTICE OF PUBLIC HEARING AS
<br />TO PROPOSED LEASE OF OFFICE SPACE
<br />In a memorandum to the Board, Ms. Pam Paluszewski, Legal Department, requested permission
<br />to advertise for a hearing for the Human Rights offices to lease office space at 319 Niles Avenue,
<br />South Bend, Indiana. Mr. Inks stated he was told that the Mayor had instructed Human Rights to
<br />look for a City -owned facility to move to, rather than renting a private office. Ms. Beachkofsky
<br />stated she would confirm whether the Notice needed to be run, but meanwhile she recommended
<br />the Board approve the request in case they did need to advertise. Therefore, upon a motion made
<br />by Mr. Inks, seconded by Mr. Gilot and carried, the above request was approved.
<br />ADOPT RESOLUTION NO. 21 -2012 — A RESOLUTION OF THE BOARD OF PUBLIC
<br />WORKS OF THE CITY OF SOUTH BEND INDIANA ADOPTING A WRITTEN FISCAL
<br />PLAN AND ESTABLISHING A POLICY FOR THE PROVISION OF SERVICES TO AN
<br />ANNEXATION AREA IN CLAY TOWNSHIP (DOUGLAS SOUTH ANNEXATION AREA)
<br />Upon a motion made by Mr. Gilot, seconded by Mr. Inks and carried, the following Resolution
