Non - Collusion Affidavit, Non - Debarment Affidavit, Employment Eligibility Verification, Non- Discrimination Commitment for Contractors and Certification of Use of United States Steel Products or Foundry Products form with their bid. The Clerk was instructed to notify them of this discrepancy and let them know that they need to submit the form within 24 hours. Upon a motion made by Mr. Gilot, seconded by Mr. Inks and carried, the above bids were referred to Engineering for review and recommendation, AWARD BID AND APPROVE CONTRACT — DIAMOND AVENUE TRUNK STORM SEWER BID PACKAGE 2 — PROJECT NO, 109 -033B 2011 SEWER BOND Mr. John Engstrom, Engineering, advised the Board that on February 28, 2012, bids were received and opened for the above referenced project. After reviewing those bids, Mr. Engstrom recommended that the Board award the contract to the lowest responsive and responsible bidder HRP Construction, PO Box 266, South Bend, Indiana 46624, in the amount of $3,522,025.00 (Base Bid Only). Therefore, Mr. Gilot made a motion that the recommendation be accepted and the bid be awarded and the Construction Contract approved as outlined above. Mr. Inks seconded the motion, which carried.
