When the final <br />decision is made, the MRO shall then report to the employer. <br />The MRO shall notify each employee who has a confirmed <br />positive test that the employee has seventy-two ( 7 2 ) hours in which <br />to request a test of the Split saMDle. If the spl it sample is <br />unable to confirm the presence of the drugs found in the primary <br />sample, or if the split samole is unavailable, the MRO shall cancel <br />the test and report cancellation and the reasons for it, to the DOT. <br />the employer, and the employee. If there is an additional cost <br />associated with the testing of the split sample, it shall be raid <br />by the employee. <br />If the MF.10 is unable to contact the employee directly, the MRO <br />shall contact a designated management official of the employer to <br />arrange for the employee to contact the MRO prior to going or) duty. <br />The MPO may verify positive results with the employer official <br />without having communicated ON the employee under the followinq <br />circumstances: <br />1 ) The employee expressly declines the opportunity to discuss <br />the results, or <br />2) Within five (5) days after a documented contact by a <br />designated management official of the employer instructing <br />the employee to contact the MRO, the employee has not done <br />so. <br />Y V T regulations recuire that. the MRO provide Ej signed <br />written notification within three (3) business days of completion <br />of the review. The MRO must reoort the following information to <br />the employer: <br />1) That the controlled substances test being reported was in <br />accordance with oart 40 of the D.O.T. regulations; <br />2) The name of the individual for whom the test results are <br />being reported: <br />3) The tyre of test indicated on the custody and conti-01 form: <br />41 The date and location of the test collection; <br />5) The identities of the entities performing the collections, <br />analysis of the Specimen and the MRO for the specific test; <br />G) The verified results of controlled substances test, either <br />positive or negative and if positive, the identity of the <br />controlled substances for which the test was verified <br />positive. <br />
