7 <br />11) Within four (4) minutes after obtaining the specimen, <br />collection site oe r son ne I sh a I I test measure; the temperature <br />of the specimen, the acceotable temperature range is 90 -100 <br />degrees F. <br />12) Immediately after the specimen is collected, the collection <br />site personnel shall inspect the specimen for color and look <br />for any sign of contamination or tampering. <br />13) If the collection site oersonnel believes tampering has <br />occurred, a second Specimen shall be collected immediately <br />under the direct observation of a same gender collection <br />site person. <br />14) Both the individual being tested and the collection site <br />personnel shall keen the soecimen in view at all times DOOF <br />to being sealed and labeled, <br />15) The collection site personnel shall place an identification <br />label securelv on the bottle containing the date, the <br />individual snecimen number and any other identifying <br />information required by the employer. The bottle shall have <br />a tamper proof tyre seal and shall be initialed by the <br />collection site person and the test subject. <br />16) The collection site personnel shall enter all information <br />identifying the specimen on the custody and control form <br />certifying collection was accomplished in accordance with <br />the instructions r)rovided. <br />17 ) The test subject shall sign the form indicating the sDecimen, <br />collected was his or hers and may also be required to sign <br />a consent or release form. <br />113) The collection site personnel shall complete the chain of <br />custody portion of the custody and control form certifying <br />proper Completion of the collection. <br />19) If the specimen is not immediately shipped to the <br />laboratory, it shall be secured during temporary storage. <br />201) The urine specimen and custody document must be under the <br />control of the collection site person while the chain of <br />custody procedures are being performed. <br />The collection site personnel shall then arrange to ship the <br />collected specimen to the drug testing laboratory in a contailier <br />with a signed tamper proof seal. The collection site personnel <br />shall ensure that the chain of custody forms are sealed inside of <br />the shipping package. <br />
