MEMORANDUM OF UNDERSTANDING <br />BETWEEN THE SOUTH BEND BOARD OF PUBLIC WORDS AND <br />OAKLAWN PSYCHIATRIC CENTER <br />This Memorandum of Understanding (this "Agreement") is made and executed between the <br />City of South Bend, Indiana (the "City") acting by and through its Board of Public Works <br />("BPW"), and Oaklawn Psychiatric Center ("Oaklawn"). <br />WHEREAS, Oaklawn provides mental health and addiction treatment for children, <br />adolescents and adults in St. Joseph County and beyond; and <br />WHEREAS, Oaklawn, the Michiana Opioid Task Force, and Beacon -Memorial Hospital <br />have secured funding from the 21" Century Cures Act for the operation of a certified peer <br />addiction recovery coaching program in Memorial Hospital and sites across the South Bend <br />community; and <br />WHEREAS, the City believes the provision of certified peer addiction recovery coaching <br />services is a useful and effective intervention in the opioid addiction crisis causing preventable <br />deaths across our community; and <br />WHEREAS, the City desires to provide funding to Oaklawn to assist with the provision of <br />these services, <br />NOW, THEREFORE, BPW and SJCHD do hereby mutually agree as follows: <br />1. TERM <br />This Agreement shall commence on October 17, 2018 and continue until Oaklawn <br />certifies to the City that the aforementioned funds have been disbursed, which shall be no later <br />than January 30, 2019. <br />H. GENERAL TERMS <br />BPW shall authorize a donation to Oaklawn in the amount of Twenty -Five Thousand <br />Dollars $25,000 (the "Donation Amount") to be used for reimbursement of certification and <br />associated costs for the "Crisis to 90 Days" peer addiction recovery coaching program. <br />III. RESPONSIBILITIES OF THE CITY <br />a) The City, through BPW, will authorize and provide the Donation Amount to <br />Oaklawn in a manner reasonably described by Oaklawn. <br />b) The City shall appoint a contact person from its Department of Community <br />Investment to confer with Oaklawn and ensure that the Donation Amount is used <br />for purchases described above. <br />c) The City agrees to obey all applicable local, state and federal laws and <br />regulations. <br />
