Introduction '
The 2019 Action Plan is prepared in consultation with community, governmental, public housing
authorities, and nonprofit service and neighborhood organizations. Several federal, state and regional
sources for information on economic, health, assisted housing, and social service issues are
utilized. Dialogue with local sub-grantees,the St.Joseph County Homeless Regional Planning Council
(RPC), and the Balance of State Continuum of Care (BoS) is on-going.
Provide a concise summary of the jurisdiction's activities to enhance coordination between '
public and assisted housing providers and private and governmental health, mental health
and service agencies (91.215(1)). 1
The City of South Bend has a seat on the RPC and attends regular monthly meetings. The Planning
Council continues to be very active in working cooperatively to apply for relevant grants, and to assess
its funding opportunities/challenges.
Regular RPC participants include: '
• Oaklawn Psychiatric Center, which serves the severely mentally ill;
• AIDS Ministries/AIDS Assist, which serves HIV/AIDS patients;
• Youth Service Bureau, which serves unaccompanied youth under the age of 25;
• YWCA, a domestic violence shelter;
• Dismas House, which serves ex-offenders;
• Hope Ministries, which serves homeless persons/families;
• Brightpoint, which works to eliminate conditions of poverty;
• The Center for the Homeless, which serves homeless persons/families;
• Life Treatment Centers, which treats persons with substance addictions; I
• The Veterans Administration, which serves the veteran population; and,
• St. Margaret's House, which is a day shelter for women.
In addition, regular meetings and/or discussions take place with representatives of local housing
authorities, Bridges out of Poverty,the St.Joseph County Health Department, and neighborhood '
organizations as needed.
On behalf of the former St.Joseph County Continuum of Care, the City of South Bend contracted with I
the Corporation for Supportive Housing's (CSH) National Consulting Team to explore the action steps
needed to bring the Continuum into compliance with current federal guidance and regulations. In
addition to requesting analysis of current administrative and oversight compliance, the Continuum
expressed interest in exploring the option of joining the Indiana Balance of State Continuum of Care.
Annual Action Plan 6
2019
OMB Control No:2506-0117(exp.06/30/2018)
