DESCRIPTION OF DOCUMENTS INCLUDED IN YOUR AWARD PACKAGE <br />The following is a list of the forms/documents that may be included within your agreement packet and an <br />explanation of their purpose(s): <br />o Authorized Contact Form— Provides IHCDA contact information for individuals that you are authorizing <br />IHCDA to contact regarding your organization's award. <br />m Authorized Signature — Provides IHCDA with the list of individuals that are authorized by your <br />organization to execute agreements and binding forms on behalf of your organization. <br />o ACH Authorization Form — Provides your organization's bank account information to facilitate the transfer <br />of funds. <br />® Award Agreement --- Binds you and your organization to the requirements associated with the award. <br />p Exhibit D (HUD Form 4010j — Explains requirements regarding Davis Bacon, including requirements for <br />sub --contractors involved inapplicable projects. <br />o HUD Disclosure Form — Used in CDBG-D grants to prevent fraud, waste, abuse, and conflicts of interest. <br />• W-9 — Collects tax information so that IRS reporting requirements can be met. <br />• Payroll Authorization Fora -- Provides to IHCDA a list of each employee being paid out by this award, <br />along with their hourly rate of pay. <br />• Drug Free Certification —Serves as a binding agreement that your organization will provide a drug free <br />workplace. <br />• Resolution — Provides evidence that your organization consented to a resolution involving the application <br />and receipt of funding from, IHCDA: <br />Which Resolution Form Generally with Some Exception <br />o Short Form: Use if the Board has met prior to the time the application was submitted and <br />approved the actions that were going to be taken. <br />o Lqi g: Use if the Board must convene after the application was submitted to approve the receipt <br />of the funds and pass the resolution. <br />[00016885-2} <br />
