(2,120') from Michigan, East along Chippewa to Erskine <br />Maintenance. <br />• Install Tracer and Pull Rope. <br />• Install (5) new handholes. <br />• Install Corning 72-strand Fiber. <br />• Install new fiber panel and splice into the Backbone. <br />• Tie conduit into existing HH. <br />• Proposal does not include Indiana sales tax. <br />• All work quoted first shift 7:100 AM to 3:30 PM <br />We hereby propose to furnish material and', labor in complete accordance with the above <br />specifications for the sum of $44,940.00 (Forty -Four Thousand Nine Hundred Sixty Dollars). <br />All materials are guaranteed to be as specified. All work to be completed in a satisfactory <br />manner according to standard practices. Any alteration or deviation from the above <br />specifications or involving extra costs will be executed only upon written orders, and will become <br />an extra charge over and above this proposal. All agreements are contingent upon strikes, <br />accidents, or delays beyond our control. The Owner will carry fire, tornado, and other necessary <br />insurance. Ourworkeirs are fully covered by Worker's Compensation Insurance. This proposal is <br />valid for 30 days from issue date. <br />ACCEPTANCE OF PROPOSAL <br />The above prices, specifications, and conditions are satisfactory and are hereby accepted. SJVM, Inc. is <br />authorized to do the work as specified.
