<br />St. Joseph Streetscape Improvements

<br />Date: August 24, 2018
<br />To: All holders of the contract document for the above project.
<br />The items in this addendum are to become a part of the original documents as if included therein. Only
<br />the noted items are affected, and the remainder of the original contract document is to remain in its
<br />entirety.
<br />This Addendum modifies the Contract Documents as follows:
<br />PART I — CLARIFICATIONS

<br />PART I - CLARIFICATIONS
<br />1.01 Attached are the meeting minutes from the Pre -bid meeting held on August 21, 2018.
<br />PART 2 — SPECIAL PROVISIONS
<br />2.01 SPECIAL PROVISIONS, Page 13, Section XX1I, Pre -Construction Video of Site Conditions:
<br />1. REVISE Item B, No. 1 to read "The Contractor shall be required to submit a professionally
<br />completed DVD recording of the project area to clearly show driveways, street pavements,
<br />signs, traffic signals, etc. prior to the start of construction to verify before and after
<br />conditions. The DVD shall indicate the date of the recording and also include a method to
<br />identify the street or area being videotaped such as stationing or some other unique
<br />identifier."
<br />2.02 SPECIAL PROVISIONS, Page 19, Section XXXV, Permits:
<br />1. DELETE Item B, No. 1 note.
<br />2.03 SPECIAL PROVISIONS, Page 19, Section XXXV, Permits:
<br />1. DELETE "Part B" callout from Item B, No. 6, a.
<br />2.04 SPECIAL PROVISIONS, Page 21, Section XXXIX, Pavement, Remove:

<br />2.06 SPECIAL PROVISIONS, Page 24, Section XLIII, Common Excavation:
<br />1. REVISE Item B, No. 12, a, to read "Payment for all costs of "Common Excavation," shall
<br />be only for specific neat line volumes as approved by the Engineer for any common
<br />excavation required during the work which is not included in any established pay item
<br />including the cost of all materials, equipment, tools, labor, transportation, operations, and all
<br />other incidentals, shall be paid for on a cubic yard basis for all parts under the pay item"
<br />2.07 SPECIAL PROVISIONS, Page 28, Section L, Hot Mix Asphalt Pavement:
