(LFA) is pleased to submit this proposal to provide Professional Engineering Services to the City of South Bend (City) for the Main Street to Lafayette Boulevard Connector and Street Improvements. The proposal includes the following documents: • Scope of Additional Services, Appendix A • Manhour Justification, Appendix B • Schedule, Appendix C. The proposal was prepared at your request on the basis of our meetings on May 16, 2011 and September 16, 2011. It includes work items and work effort not envisioned or included in our original proposal. Also, we have made appropriate deductions for some items that will not be needed for the completion of design services. We propose this Scope of Services be performed on an hourly basis for a budgetary fee of $260,320.00. Payments to LFA will be in accordance with Attachment "A" in our original contract. Please indicate your acceptance of this proposal by signing in the space provided below and returning one copy to our office.
