September 2011 <br />City of South Bend <br />Proposal for Professional Services uo�wuc <br />Prairie Avenue Fire Station Page l of 4 <br />APPENDIX A <br />SCOPE OF SERVICES <br />Protect Description <br />The proposed project consists of construction a new fire station on Lot 2 of the <br />Hollingsworth Minor Subdivision, west of Bruno's Pizza on State Road 23 / Prairie <br />Avenue, to replace the existing station down the street. The proposed site is currently <br />zoned LB in the City of South Bend and would require a Special Exception Use. <br />Scope of Services <br />Based upon our understanding of the project goals and objectives, we propose to <br />perform the following services to assist the City with completing this project: <br />Task #1: Boundary Survey— New Parcel <br />This work includes locating the existing and proposed lot comers in the field initially with <br />lathe for your review with the seller. Once the final parcel configuration is determined, <br />we will complete a boundary survey in accordance with the ALTA minimum standard <br />detail requirements and set irons. <br />Task #2: Phase One Environmental Report — New Parcel <br />(Sub - Consultant: Weaver Boos) <br />This task involves performing a Phase One ESA of the New Fire Station Property in <br />accordance with ASTM E1527 -05 (adopted in 2006) and generally accepted industry <br />standards concerning environmental site assessments. A written report shall be <br />prepared that lists any recognized environmental conditions identified, consolidate <br />and discuss information gathered from background research and site investigation, <br />and include any significant supporting documenbtion. <br />Task #3: Replat — New Parcel <br />This work includes preparing a replat drawing and application to move the existing <br />property lines to the new location if the entire parcel is not acquired. <br />Task #4: Special Use Exception — New Parcel <br />This work includes an application to the Board of Zoning Appeals to allow for a fire <br />station use in LB zoning. <br />Task #5: Boundary Survey — Existing Station Parcel <br />This work includes locating or setting irons on the existing fire station parcel and <br />completing a boundary survey in accordance with the ALTA minimum standard detail <br />requirements. <br />Engineers Planners Surveyors <br />
