REQUIRED PROGRESS REPORTS Federal Agency: U.S. Department of Housing and Urban Development Federal Program: Neighborhood Initiative Grants CFDA Number: 14.Unknown Federal Award Number: B-01-NI-IN-SB-001 The effective date of the grant agreement was May 16, 2001. No progress reports on performance and financial status were prepared or filed in 2001 or in 2002. The grant agreement sets out the following provisions for reporting: "ARTICLE IV. Progress Reports. The Grantee shall submit a progress report every six months after the effective date of the grant agreement. Progress reports shall include reports on both performance and financial progress and shall conform with 24 CFR 85.40 and 85.41 or CFR Sections 84.50 through 84.53, as applicable. Additional information required or increased frequency of reporting as may be described in Article VI11(C). A. The performance reports must contain the information required under 24 CFR Part 85.40(b) (2) or 24 CFR Part 84.51(a), as applicable including a comparison of actual accomplishments to the objectives indicated in the approved application, the reasons for slippage if established objectives were not met, and additional pertinent information including explanation of signifi- cant cost overruns. B. Financial reports shall be submitted on Standard Form 269A. C. No grant payments will be approved for projects with overdue progress reports." Failure to submit reports as required may result in delays in receiving grant funds and possibly they loss of future grant funds. All required progress reports for performance and financial status should be filed every six months as required by the grant agreement. Officials concur and have filed all the past required reports on March 31, 2003. 154
