discharge, �New sSources, and sources that become Categorical }Industrial �Users subsequent to the promulgation of an applicable categorical sStandard, sha11 be required to submit to the Director a report which contains the information listed in subparagraph (�} � below. A �New sSource shall also be required to report the method of pretreatment it intends to use to meet applicable pretreatment standards. A new source shall also give estimates of its anticipated flow and quantity of pollutants discharged. �-} a. �° �� tUsers described above shall submit the information �°a '��.+'�;� °°^+;^^ �n�=�� set forth below: �.� Identifying information. The name and address of the facility including the name of the operator and owners to�ether with contact information, description of activities, facilities and plant production processes on the premises. �. ii Environmental control. A list of any environmental �e�permits held by or for the facility. s. iii Description of operations. A brief description of the nature, average rate of production (including each product produced by type amount processes and rate of roduction , and standard industrial classifications of the operation(s) canied out by such �� User. This description should include a schematic process diagram which indicates points of discharge to the POTW from the regulated process. � iv Flow measurement.
