May, 2011 <br />City of South Bend <br />621 East Jefferson Boulevard <br />Streetscape Modernization Page 1 of 3 <br />APPENDIX A <br />Work Plan <br />PROJECT SCOPE <br />The proposed project consists of the streetscape modernization from 621 East Jefferson <br />Boulevard. The improvements will include replacement of the existing approaches and <br />sidewalks, addition of a landscaping planter, and improved drainage by use of a <br />pervious concrete sidewalk. The project will be designed in accordance with the <br />planned streetscape modernization project which will include Jefferson Boulevard from <br />Niles Avenue to St. Peter Street. <br />SCOPE OF SERVICES <br />The scope of services proposed herein includes the design and development of quote <br />documents for the project and are further described as follows: <br />Task #1: Design: <br />a) Revise City of South Bend Title Sheet <br />b) Revise typical section sheets <br />c) Revise grading plan <br />d) Revise plan view sheet of improvements <br />e) Revise pervious concrete detail sheet <br />f) Revise maintenance of traffic plan <br />g) Revise landscape plan <br />h) Coordinate with utility companies <br />1) Calculate project quantities <br />j) Develop cost estimate <br />k) Submit three (3) sets of plans to the City for review <br />1) Meet with the City to review the plans <br />m) Revise plans according to the City review <br />Task #2: Quote Documents: <br />a) Prepare bid proposal sheets <br />b) Prepare special provisions <br />c) Prepare Quote Package <br />d) Submit three (3) sets quote documents to the City for review <br />e) Meet with the City to review the quote documents <br />f) Revise documents according to the City review <br />Engineers Architects Planners Surveyors <br />
