D. Williams, President and CEO Executive Director of Family&Children's Center <br /> Properties Management, Inc. and Ann E. Kolata, Deputy Executive Director of the South Bend Redevelopment <br /> Commission, make application and petition the Common Council of the City of South Bend, Indiana to amend <br /> the zoning ordinance as herein requested. <br /> 1) The property sought to be rezoned is located at: <br /> On Fellows Street at the intersection with vacated Keasey Street as shown on the <br /> attached E�ibit "A", consisting of two parcels, Parcel "B" being owned by <br /> Family & Children's Center Properties Management, Inc. and Parcel "C" being <br /> owned by South Bend Redevelopment Commission. <br /> 2) The name and Address of the Property owner is: <br /> Family & Children's Center Properties Management, Inc., 1411 Lincolnway <br /> West, Mishawaka, Indiana 46544, as to the northerly portion of the property <br /> described as Parcel "B" on Exhibit "A" attached hereto and The South Bend <br /> Redevelopment Commission, 12th Floor, County-City Building, South Bend, <br /> Indiana 46601, as to the southerly portion of the property described as Pazcel <br /> "C" on E�chibit "A" attached hereto. <br /> 3) It is desired and requested that the Property be rezoned from "B" Residential District "A" Height and <br /> Area to "PHC Professional/Health Care District". <br /> 4) The property will be purchased by Memorial Health System, Inc. which will build a medical clinic <br /> containing approximately Sixteen Thousand Square Feet (16,000 sq. ft.) of space on the property as is <br /> depicted in the preliminary site plan which is filed with this Petition. <br /> 5) Attached is a copy of (a) legal description of the property; (b) a list of names and addresses of all <br /> property owners within 300 feet of the petitioned Property (c) six (6) site plans; and addressed, stamped <br /> envelopes for all property owners within 300 feet of the subject property. <br /> 6) By signing this Petition, I and any contingent purchaser understand that if the Council approves this <br /> petition to rezone, it may be approved subject to the submittal of a final site plan. A final site plan must <br /> be submitted to the Area Plan Commission within one(1)year of the Council's action. Failure to submit <br /> a final site plan within that time period, causes the zoning of the petitioned property to revert to the initial <br /> zoning classification.
