' AP-10 Consultation - 91.100, 91.200(b), 91.215(1) <br /> ' 1. Introduction <br /> The 2018 Action Plan is prepared in consultation with community, governmental, public housing <br /> authorities, and nonprofit service and neighborhood organizations. Several federal, state and regional <br /> ' sources for information on economic, health, assisted housing, and social service issues are <br /> utilized. Dialogue with local sub-grantees and the St. Joseph County Homeless Regional Planning <br /> Council is on-going. <br /> Provide a concise summary of the jurisdiction's activities to enhance coordination between <br /> public and assisted housing providers and private and governmental health, mental health <br /> and service agencies (91.215(1)). <br /> The City of South Bend has a seat on the RPC and attends regular monthly meetings. The Planning <br /> Council continues to be very active in working cooperatively to apply for relevant grants, and to assess <br /> its funding opportunities/challenges. <br /> Regular RPC participants include: <br /> • Oaklawn Psychiatric Center, which works primarily with mental illness issues; <br /> • AIDS Ministries/AIDS Assist, which provides services to HIV/AIDS patients; <br /> • Youth Service Bureau, which services unaccompanied youth under the age of 25; <br /> • YWCA, a domestic violence shelter; <br /> • Dismas House, which serves ex-offenders; <br /> • Brightpoint, which serves to eliminate conditions of poverty; <br /> • The Center for the Homeless, which provides services to homeless persons/families; <br /> • Life Treatment Centers, which treats persons with substance addictions; and, <br /> • The Veterans Administration, serving the veteran population. <br /> In addition, regular meetings and/or discussions take place with representatives of local housing <br /> authorities, Bridges out of Poverty, and the St.Joseph County Health Department. <br /> On behalf of efforts related to the former St. Joseph County Continuum of Care, the City of South Bend <br /> contracted with the Corporation for Supportive Housing's (CSH) National Consulting Team to explore the <br /> action steps needed to bring the Continuum into compliance with current federal guidance and <br /> ' regulations. In addition to requesting analysis of current administrative and oversight compliance, the <br /> Continuum expressed interest in exploring the option of joining the Indiana Balance of State Continuum <br /> of Care. After review and consultation with CSH, the St. Joseph County Continuum of Care unanimously <br /> ' approved reaching out and pursing the process to join the Balance of State.
