REGULAR MEETING MAY 22, 2018 156 PUBLIC HEARING — NEW MASSAGE ESTABLISHMENT — SUMMER SPA INC. Mr. Gilot advised that this was the date set for the Board's Public Hearing concerning a license application for Summer Spa, Inc. He opened the Public Hearing and noted favorable recommendations were received from the Area Plan Commission, Police, Fire, and Community Investment Departments. Mr. Nick Nicolson and Ms. Junhua Guan, owners of the establishment were present and spoke in favor. There being no one else present wishing to address the Board concerning this matter, the Public Hearing was closed. Therefore, upon a motion made by Ms. Maradik, seconded by Ms. Dorau and carried, the application for a new Massage Establishment was approved. OPENING AND AWARD OF BIDS — VIETNAM VETERANS MEMORIAL RELOCATION — PROJECT NO. 117-047A (REGIONAL CITIES GRANT) This was the date set for receiving and opening of sealed bids for the above referenced project. The Clerk tendered proof of publication of Notice in the South Bend Tribune, which was found to be sufficient. The following bids were opened and publicly read: SLATILE ROOFING AND SHEET METAL CO., INC. 1703 S. Ironwood Dr. South Bend, IN 46613 Bid was signed by: Gerald Longerot Non -Collusion, Non -Discrimination Affidavit Form was completed Five percent (5%) Bid Bond was submitted BID: Base Bid $152,850 Alternate No. 1 ($4,272) C&S MASONRY RESTORATION LLC 3725 N Foundation Ct., Suite AB South Bend, IN 46628 Bid was signed by: Steve Fields Non -Collusion, Non -Discrimination Affidavit Form was completed Five percent (5%) Bid Bond was submitted BID: Base Bid $241,625 Alternate No. 1 ($1,500) ZIOLKOWSKI CONSTRUCTION, INC. 4050 Ralph Jones Drive South Bend, IN 46628 Bid was signed by: Bill Favors Non -Collusion, Non -Discrimination Affidavit Form was completed Five percent (5%) Bid Bond was submitted BID: Base Bid $281,000 Alternate No. 1 ($10,000) Upon a motion made by Ms. Maradik, seconded by Ms. Dorau and carried, the above bids were referred to Engineering and Venues, Parks, and Arts for review and recommendation. After reviewing those bids, Mr. Roger Nawrot, Engineering, recommended that the Board award the contract to the lowest responsive and responsible bidder Slatile Roofing and Sheet Metal Co., Inc., in the amount of $152,850 for the base bid only. Therefore, Ms. Maradik made a motion that the recommendation be accepted and the bid be awarded and the contract approved as outlined above. Ms. Dorau seconded the motion, which carried.
