Describe coordination with the Continuum of Care and efforts to address the needs of <br /> homeless persons (particularly chronically homeless individuals and families, families with <br /> ichildren, veterans, and unaccompanied youth) and persons at risk of homelessness. <br /> The City and SJC RPC discussed and agreed that ESG funds would be used to assist nonprofit <br /> organizations that provide assistance to homeless individuals and families by supporting the following: <br /> ' • Broadening existing shelter activities; <br /> • Emphasizing Rapid Re-housing (helping individuals and families who are literally homeless [in a <br /> shelter or sleeping in a place not meant for human habitation])to quickly access permanent <br /> housing; <br /> • Helping people quickly regain stability in permanent housing after experiencing a housing crisis <br /> and/or homelessness; <br /> • Aligning the ESG program with other HUD programs; and, <br /> • Supporting more coordinated and effective data collection, performance measurement, and <br /> ' program evaluation. <br /> Describe consultation with the Continuum(s) of Care that serves the jurisdiction's area in <br /> determining how to allocate ESG funds, develop performance standards for and evaluate <br /> outcomes of projects and activities assisted by ESG funds, and develop funding, policies and <br /> procedures for the operation and administration of HMIS <br /> The proposed performance standards are based on the regulations, desired outcomes, and experience <br /> with HPRP.The Center for the Homeless has been and continues to be the Continuum's lead agency <br /> with regard to HMIS. The Center maintains an individual as the lead point person who can assist with <br /> training and any HMIS issues, and contracts directly with Client Track for HMIS service. All HMIS users <br /> follow confidentiality and privacy requirements. <br /> i2.
