a <br />A01 <br />EXHIBIT 1: ACKNOWLEDGEMENT OF OBLIGATION TO INFORM PATIENTS OF PARTICIPATION <br />I acknowledge that as a Covered Entity, our organization is obligated to inform our patients regarding our <br />participation in health information exchange. I have received a copy of the MHIN Community View Access <br />Management Policy, HIE Patient Opt -Out Policy, and suggested wording that can be used in our Notice of <br />Privacy Practices (NPP), <br />I have also been made aware that the Office of Civil Rights (OCR) encourages healthcare providers to include <br />information about health information exchanges in their NPP. The OCR takes the position that providers <br />should be "open and transparent about any policies,, procedures, and technologies that directly affect <br />individuals and/ortheir individually identifiable health information." As noted by the OCR, healthcare providers <br />are encouraged to use their NPPs or a separate notice of the disclosures that may be made to and through a <br />health information exchange, and describe how the patient's Protected Health Information (PHI) will be <br />protected.
