Klosinski, Assistant City Engineer <br />SUBJECT: Agenda Request Form -- Change Order No. 18 <br />Project No. 114-053: Wastewater Treatment Plant Secondary Treatment <br />Improvements <br />DATE: April 16, 2018 <br />The Division of Utilities is requesting the Board of Public Works approval of Change Order No. <br />18 for Project No. 114-053 Wastewater Treatment Plant Secondary Treatment Improvements. <br />Refer to attached documents for supporting details for the subject work. <br />Change Order No. 18 addresses the following changes: <br />COP 109 — Repair Grating at FC#1 <br />$ <br />1,000.00 <br />COP 110 -- Install 24" Blind Flange at Gallery #1 <br />$ <br />1,325.00 <br />COP 111 — Additional 16 SFT of Surface Spall Repair <br />$ <br />3,296.00 <br />COP 112 — Reimburse City for Spill Expenses 12/01/2017 <br />($ <br />5,650.00) <br />COP 114 — Isolation Gate 6 & 7 Handrails <br />$ <br />496.00 <br />COP 117 — Enviromix Firmware, Credit <br />($ <br />750.00) <br />COP 123 — Revised Short Circuit Study (Electrical) <br />$ <br />1,323.00 <br />Unusued Unit Price Quantity Deductions <br />($ <br />52,219.56) <br />NET DECREASE FOR CHANGE ORDER NO. 18 ITEMS <br />($ <br />51,179.56) <br />Original Contract Sum $11,885,000.00 <br />Net change by previously authorized change orders $ 2,063,269,47 <br />The contract sum prior to this change order $13,948,269.47 <br />By this change order, the project amount decreases by ($ 51,179.56) <br />The new contract sum including this change order is $13,897,089.91 <br />This Change Order represents a total change of -0.43% <br />Total change for the entire project +16.93% <br />Time Extensions resulting from Change Order No. 18: <br />Substantial Completion: 33 days <br />Final Completion: 33 days <br />Comments: <br />28 days for delays due to the Flood Event in February 2018 <br />5 days for COP 113 and COP 116, costs to be included in Change Order No. 19. <br />
