The enclosed application, when fully completed, will have all of the <br />required information; and it lists any other materials that will be needed to process the <br />business license. Please, do not omit anything. <br />r Application submission. Please mail the application along with a $5.00 processing <br />fee (either a check or money order made out to the City of South Bend) to: <br />City of South Bend <br />Attn: Winona Hays /Licensing <br />227 W.
