With this change, Madison Center is no longer providing outpatient <br />treatment to the clients receiving assistance from the Supportive Housing Programs. Madison Center will not be <br />able to provide the supportive services that are required for these grants. Therefore,1 am requesting that the grants <br />be transferred to: <br />Oaklawn Psychiatric Center <br />Community Mental Health Center for St. Joseph Co. <br />P.O. Box 1240 <br />South Bend, IN 46624 <br />Oaklawn has assumed the supportive service requirement of the grants. The grants in question are the Supportive <br />Housing Rental Assistance (grant # IN00I2B5H000802) with a grant period of 211110- 1/31/1 l and the Supportive <br />Housing Turnock Group Home ( previous grant # INOO I 3BSH000802) with a grant period of 7/1110 - 6/30/11. <br />In addition, Madison Center is also unable to continue to be the sponsoring agency for the Shelter Plus Grants for <br />the City of South Bend.
