The scope changes and additional work are described <br />below: <br />1. Provide a topographic survey on the west side of Carroll Street from Ohio to <br />Sample and on the south side of Sample Street from the West property line <br />of 236 Sample to Carroll Street. <br />2. Provide a design for new curb, sidewalk and ADA ramps on the west side of <br />Carroll Street from Ohio to Sample and on the south side of Sample Street <br />from the west property line of 236 Sample Street to Carroll Street. <br />Additional Estimated Fee: $3,803.00 <br />This amendment is subject to the General Terms and Conditions included in the <br />original contract. <br />1 <br />
