(e} Power of Attorney. If a person other than the person signing the application is to <br />represent the applicant at any meeting of the Community and Economic <br />Development Committee or .other Council, a duly executed Power of Attorney <br />authorizing such representation must be on f le with the City Clerk. <br />Sec. 2-84.2 Statement of benefits, declaratory and confirmatory resolutions required. <br />(a) In addition to the completed petition, filing fee, and related documents required <br />by section 2-$3.1, the owners of real property or New Personal Property must file <br />a completed statement of benefits form at the time of filing the petition. <br />(b} Proposed forms of declaratory and confirmatory resolutions are also required of <br />the owner at the time of filing the petition. <br />(c) Petitioners must agree to work with the Community and Economic Development <br />Department in providing it with any additional information required for their <br />review. Petitioners must further agree that they will comply with the requirements <br />of the Redevelopment Design Review Committee where applicable, anal provide <br />information to WDS, and the Council's Community and Economic Development <br />Committee. <br />(d) The provisions of IC 6-1.1-12.1-1 et seq. shall be followed by the designating <br />body when reviewing such documents required by this section. <br />Sec. 2-84.3 Petitioner's Claims for Phase-In Credits, <br />(a) The granting of a credit for real and/or personal property tax abatement <br />shall be governed under the terms and conditions of this Article including <br />the current Schedules found in Division 2 above. <br />(b) In the Statement of Benefits and the Compliance with Statement of <br />Benefits Real Estate Improvements (Form SB-llReal Property and Form <br />CF-1/Real Property), and in the compliance with Statement of Benef is <br />Personal Property (Form CF-1/PP), Petitioner shall execute a statement <br />under the pains and penalties for perjury that the items disclosed in the <br />forms are true and correct, and the Petitioner is only claiming the Phase-In <br />Benefits permitted under this Article and the Schedules attached thereto. <br />(c) Accordingly, if the Petitioner fails to accurately describe the Phase-Ins to <br />which the Petitioner is entitled, and the activity which justifies the Phase- <br />In Credits, the person executing the forms shall be subject to the penalties <br />for perjury under the laws of the State of Indiana. <br />{d) The Community and Economic Developmen# Department shall utilize <br />information required to be filed by the Petitioner in the SB-1, CF-I, and <br />19 <br />
