CR-05 - Goals and Outcomes <br /> ' Progress the jurisdiction has made in carrying out its strategic plan and its action plan. <br /> 91.520(a) <br /> ' This could be an overview that includes major initiatives and highlights that were proposed and <br /> executed throughout the program year. <br /> ' In South Bend during 2016, 66 households received help with housing issues: 33 homeowners were <br /> assisted with repairs, and 33 potential homebuyers received homebuyer education. Low-mod income <br /> homeowners received assistance with needed repairs through the following PY 2016 programs: SBHIP <br /> ' and Rebuilding Together. <br /> ' The Community Homebuyers Corporation (CHC) assisted 12 low-mod income buyers purchase their first <br /> home. <br /> In addition,the City of South Bend supported affordable housing efforts through four major partners: <br /> South Bend Heritage Foundation (SBHF) and the Near Northwest Neighborhood, Inc. (NNN), the <br /> Northeast Neighborhood Revitalization Organization (NNRO), and Habitat for Humanity of St.Joseph <br /> County. <br /> SBHF has developed affordable housing for the residents of South Bend for the past 40 years. SBHF <br /> currently focuses its efforts in the Near West Side of South Bend. In 2016 SBHF completed the following <br /> projects: <br /> • Rental units rehabbed at 905-907 Lincolnway West; <br /> • Single-family home rehab at 115 Garfield; <br /> ' • Solar panels installed in four(4) new construction single-family homes in Lincoln Park <br /> SBHF projects to be completed in 2017: <br /> • Single-family home rehab at 121 Walnut; <br /> ' • Single-family home rehab at 225 N.Taylor; <br /> • Single-family home rehab at 816 W. Colfax; <br /> • Single-family home rehab at 1120 W.Jefferson; <br /> • Single-family home new construction at 915 Oak; <br /> • Single-family home new construction at 928 Oak. <br /> The NNRO formed in 2000 to strengthen and revitalize South Bend's Northeast Neighborhood. In 2016, <br /> one (1) low-mod household received mortgage assistance through the NNRO's HOME-funded Mortgage <br /> ' Investment Program. <br /> The NNN has been improving the quality of life in the Near Northwest Neighborhood for the past 20 <br /> CAPER 1 <br /> OMB Control No:2506-0117(exp.07/31/2015) <br />
