Submit the following required documentation and invoice <br />by the fifth (5th) day of each month: <br />1. Verification of client loan source; <br />2. Trial or Permanent Loan Modification Verification; <br />3. Authorization Form; <br />4. Disclosure Form; <br />5. Privacy Policy; <br />6. Verified Household Budget; <br />$300,00/client <br />Completion of <br />7. Documentation of back end DTI ratio; <br />Level 4a <br />8. Action Plan (If these items are part of the <br />counselor's notes, NFMC requires that the information <br />be transferred to a form titled 4a Action Plan so that <br />the assessment and course of action are clearly defined <br />for the client and for compliance testing.); <br />9. Counsel on Staying Current; <br />10. Referrals; <br />11. Date of follow-up meeting; and <br />12. Certification that each client is an owner -occupant of <br />his or her home. <br />Submit the following required documentation and invoice <br />by the fifth (5'h) day of each month: <br />1. Documentation of back -end DTI at time of second <br />Level 0appointment; <br />5. Update on Verified Household Budget; <br />6. Status of client's modified loan; <br />7. Progress against the Action Plan; and <br />8. Certification that each client is an owner -occupant of <br />his or her home. <br />Unless modified in writing and signed by the parties in the same manner as the Contract to which <br />this Exhibit B is attached, the total amount of fees under this Contract shall not exceed Eleven <br />Thousand Five Hundred Seven and 00/100 Dollars ($11,507.00). <br />(00028595-11 <br />Page 35 of 39 <br />
