Cell phone number of sponsor which will be accessible throughout the special event. <br />(b) Application Fee for Special Events at Boarding Houses: A fee of five dollars ($5.00) shall <br />be paid at the time of filing the completed application. Such application forms shall be available from -the <br />Office of the Licensing Auditor, and shall also be available on-line from the City of South Bend official <br />website. <br />(c) Deadline for Filing and Processing Application for Special Events at Boarding Houses: All <br />applications required by this section must be submitted to the Office of the Licensing Auditor for <br />processing, not less than five (5) business days prior to the special event. The City shall have the authority <br />to waive the five (5) day filing provision in this section in the event of an emergency.s <br />(d) Review and Recommendation. Upon the filing of a completed application, it shall be referred <br />to the Police Department and the Fire Prevention Bureau for their review and recommendations and then <br />returned the Office of the Licensing Auditor in the Department of Administration and Finance. <br />(e) Conditional Use Permit for a Special Event at Boarding House. Upon the City's approval <br />of a special event at a boarding house where twenty-five (25) or more persons are invited to attend and <br />would have access to alcoholic beverages of any kind, the sponsor shall pay a fee of fifteen dollars <br />($15.00) and may be issued a "Conditional Use Permit for Special Event at a Boarding House" by the <br />Department of Administration and Finance. Said permit must be conspicuously displayed in the inside <br />front window of the location of such special event. <br />(fJ Conditions: The sponsor shall have the duty to take all reasonable steps to prevent the <br />consumption of alcoholic beverages of any kind by any minor at the special event at the boarding house. <br />Reasonable steps include: <br />1. Controlling the access to alcoholic beverages of any kind at the special event; <br />2. Controlling the quantity of alcoholic beverages present at the special event; <br />3. Verifying the age of persons attending the special event by inspecting drivers' licenses or other <br />government-issued identification cards to ensure that the minors do not consume alcoholic <br />beverages of any kind while at the special event; and <br />4. Supervising the activities of minors at the special event. <br />Nothing in this sub-section should be interpreted to prohibit any religious practice which includes the use <br />of alcohol.
